To be filled out by parent(s) or guardian(s) whose child(ren) need medication administered during daycare by provider.  Medications cannot be dispensed without these consent forms.   Form 9221 must be filled out when parents request OTC products (i.e.: Tylenol, Motrin, sunblock, diaper rash ointment, medicated powders, etc.) are to be administered to your child by his/her daycare provider -- NO EXCEPTIONS!
